We collect information you provide directly to us, including: name, email address, phone number, health and medical information provided through intake questionnaires and clinical encounters, insurance information, payment information, and communications with our care team.
We also collect information automatically when you use our Services, including: log data (IP address, browser type, pages visited), device information, and cookies and similar tracking technologies as described in our Cookie Policy.
We do not sell your personal information. We share information with: licensed providers in our network (for treatment); technology partners operating under HIPAA Business Associate Agreements (Zoom for Healthcare, Surescripts, Availity, Epic MyChart, Stripe); and as required by law or for legal compliance.
We retain your health records for the minimum period required by applicable state and federal law, which is typically 7 years for adult medical records from the date of last service.
